If you have a child over age 18, it is important that they have an Advance Health Care Directive. This form allows your child to choose an agent to make medical decisions for him or her if he or she were not able to so. The second part to the form allows your child the opportunity to choose their preferred life-saving treatment options. If your child is away at college, this is the one form you must have on hand if an emergency situation were to ever occur.
